Senior Pharmacist

- Ensure implementation of the pharmaceutical care plan by assuring the accurate and timely distribution of medication to patients. Serve as drug information resource for pharmacy personnel, medical and nursing staff, as well as other healthcare professionals. Perform professional work in the compounding and dispensing of medication in accordance to Pharmacy Department's established policies and procedures. Actively participate in patient care rounds and meetings as an integral part of a multidisciplinary team. Review and interpret medication orders for clarity, accuracy, rationality, compatibility, feasibility and suitability for the patient's specific need. Review the patient's chart and performs daily follow-up. Recommend, monitor, evaluate, and assumes other responsibilities for patient-specific pharmacotherapy plan to ensure optimal outcomes. Assist medical staff with the selection of drug therapy and patient monitoring. Suggest appropriate, cost-effective therapeutic alternatives to medical staff, as needed. Provide accurate, clinically-relevant drug information to pharmacy, medical and nursing staff, as well as other healthcare professionals Conduct research to further pharmaceutical care and advance the treatment and prevention of disease, as needed. Provide education and counseling to patients and their families about medications specific to the patient's disease state. Document therapeutic recommendations in the patient's medical record and pharmacy intervention forms. Detect, analyze and report adverse drug reactions.
